Another take on the CNC Tool Holder w/some textual info to make finding the right bit easier.

Notes

Created using Carbide Create. Using 3 bits: 1/8th, 1/4th, VCarve bit. Material is 3/4" MDF. I sprayed (lots of coats) prior to carving. I used Spektra Bits (46200K, 46202K) as well as RC-1148.
